Please read the following terms carefully. By booking a session, you agree to these conditions. If you do not agree, please do not schedule or proceed with any sessions.

INITIAL INTAKE
​
All clients are required to complete a comprehensive intake form at the time of booking their session. This intake form replaces the former 15-minute discovery call and is a required step to ensure that Rapid Transformational Therapy (RTT) or Quantum Healing Hypnosis Technique (QHHT) is an appropriate fit for the client’s needs.
The intake form gathers important information, including but not limited to:

Current goals and intentions for the session

Relevant medical or psychological diagnoses

Medications or conditions that may fall outside the scope of RTT or QHHT

Background information necessary to prepare for the session
Clients must provide accurate, complete, and honest information. This information is reviewed prior to the session to confirm eligibility, ensure practitioner preparedness, and uphold ethical scope of practice.
No therapy takes place during the intake or review process. Any discussion of expected outcomes or estimated number of sessions is based solely on the information provided at the time and is not guaranteed. These estimates may change as the therapeutic process unfolds.
Stargate Healing Hypnosis reserves the right to postpone, reschedule, or decline a session if the intake information indicates that RTT or QHHT is not appropriate or if additional support outside this modality is recommended.

BOOKING PAYMENT
​
All sessions—both in-person and online—must be secured at the time of booking. Clients may choose to pay in full or select from one of the available payment plan options offered at checkout.
​
No appointment is confirmed or held without an active payment or payment plan in place. This policy ensures mutual respect, energetic commitment, and dedicated preparation time for your session.
​
CANCELLATION, RESCHEDULING & REFUNDS
If you need to cancel or reschedule your session, please notify us as soon as possible by emailing us at stargatehealinghypnosis@gmail.com. We strongly recommend providing at least 48 hours’ notice so we can plan accordingly. This policy applies to all sessions, both in-person and online.

Cancellations made with 48 hours’ notice or more: If the cancellation occurs prior to practitioner payout processing, no penalty is applied. Refunds are only available for eligible cancellations made before practitioner payouts are processed. Once session fees have been distributed to the practitioner, refunds are no longer available.

No-shows: If a client does not arrive or join the session within 15 minutes of the scheduled start time and has not made contact via email, the appointment will be considered a no-show. No-show appointments are non-refundable, and 100% of the session fee will be retained.

Rescheduling: If you wish to reschedule rather than cancel, your payment may be applied toward a future session, provided a new session date is selected at the time of rescheduling. Rescheduling requests without a confirmed new date will be treated as a cancellation and subject to the above policy.

Refunds are issued in accordance with our cancellation policy. Session fees reflect the time, preparation, and professional expertise reserved for each client. While every session is conducted with the highest level of care and intention, individual results cannot be guaranteed.

No refunds are provided for completed sessions, regardless of outcome. If a discounted package or multi-session program is purchased and you choose to discontinue before all sessions are used, a pro-rated refund may be issued. This refund will be calculated by deducting the full standard (non-discounted) rate for any sessions already completed, plus any applicable processing fees.

Session Fees & Payment Methods: All fees will be clearly communicated prior to booking. Rates are subject to change, but you will always receive confirmation of the current fee at the time of booking. All payments must be made online via a credit, debit card, or other 3rd party platforms presented at check-out. Cash, checks, or in-person payments are not accepted.
STAY IN TOUCH

All communication outside of your scheduled session will be via email at stargatehealinghypnosis@gmail.com, and will be responded to during standard office hours: Monday–Friday, 10:00 AM–7:00 PM (Central Time). Messages sent outside of these hours will be reviewed and responded to during the next available office hours.

MEDICAL OR PSYCHOLOGICAL CONDITIONS
​
To ensure client safety and the appropriateness of each session, we require information about medical history and current health conditions. This information is essential to the flow of the session, ethical preparation, and to confirm that the client falls within our scope of practice.
​
Clients agree to disclose any relevant medical, psychological, or health-related conditions honestly and to inform us of any changes to their health status throughout their time working with us, including when returning for sessions after a break.
If a client is currently under the care of a healthcare provider (such as a general practitioner, therapist, psychologist, or psychiatrist), we may require written consent from that provider before proceeding with sessions.

Please note: We are unable to offer sessions to individuals with epilepsy, psychosis, or dissociative and personality disorders, in accordance with professional, ethical, and safety standards. We reserve the right to decline or discontinue services at any time if a practitioner determines that proceeding would be outside their scope of practice or not in the client’s best interest.

SESSION GUIDELINES & EXPECTATIONS
​
Session Attendance: Please arrive on time for your scheduled session. If you anticipate being late, contact us as soon as possible at stargatehealinghypnosis@gmail.com. While we will do our best to accommodate the remaining session time, a full session cannot be guaranteed if it impacts subsequent appointments.
​
If a client does not arrive or join the session within 15 minutes of the scheduled start time and has not made contact, the appointment will be considered a no-show. No-show appointments are non-refundable, and 100% of the session fee will be retained. This policy applies to all in-person and online sessions.
​
Respectful Environment: Our practitioners are committed to maintaining a safe, respectful, and professional environment during all sessions. Clients will always be treated with compassion, respect, and integrity, and the same is expected in return. Clients must not attend sessions under the influence of alcohol or recreational drugs, with the exception of prescribed medications taken as directed. Any form of abusive, harmful, disruptive, or inappropriate behavior may result in the immediate termination of the session without a refund, and future sessions may be declined at our discretion.

Session Outcomes: While our practitioners apply their professional training, experience, and best efforts to support clients in reaching their goals, therapeutic outcomes cannot be guaranteed. Each client’s level of commitment, openness, and participation plays a vital role in the process and influences the overall experience and results.
​
Hypnosis Recordings: If a hypnotherapy recording is provided as part of a session, it is intended for the client’s personal use only and may not be shared, copied, reproduced, or distributed in any form.
For safety reasons, these recordings must not be listened to while driving, operating machinery, or engaging in any activity that requires full attention and alertness.

CONFIDENTIALITY & CLIENT CONSENT
​
All communications and sessions between you and Stargate Healing Hypnosis will be treated with the strictest level of confidentiality. This includes all verbal, written, and electronic correspondence, as well as session recordings, notes, and intake forms.
​
Your Confidentiality Rights: Unless otherwise required by law or ethical obligation (outlined below), no information you share— whether through consultation, intake forms, emails, text messages, or live sessions—will be disclosed to any third party without your explicit, written consent. All client files, records, and recordings are stored securely in password-protected digital files or in locked physical storage units, and are accessible only by the practitioner. Recordings may be taken during your session solely for your personal integration and reference, or for professional review by the practitioner.
Exceptions to Confidentiality: There are specific situations in which the practitioner is legally and ethically required to break confidentiality. These include:

With your explicit written permission, if you choose to share your experience for educational or promotional purposes, as described in the Social Media & Public Sharing Consent form.

Court-ordered disclosure – if required to present information by subpoena or other legal mandate.

Threats to safety – if there is reason to believe that you or someone else is at risk of serious harm.

Criminal activity – including instances of abuse, neglect, fraud, or criminal behavior, particularly where minors (under the age of 18) or vulnerable adults are involved.

Healthcare collaboration – if you are referred by a licensed medical professional (e.g., a GP, psychiatrist, or other healthcare provider) and a progress report is required. You may request a copy of this report.

Optional Social Media & Public Sharing Consent: You will have the option, during your intake process, to opt-in or opt-out of allowing your session experience (audio and/or written narrative) to be included anonymously on Stargate Hypnosis' public platforms (such as Instagram, YouTube, or podcast formats).

If you opt-in, here’s what that means:

Your identity will never be disclosed.

Any identifying details (names, locations, etc.) will be altered or removed to preserve your anonymity.

You may revoke this consent at any time in writing.

If you do not opt-in, your session recordings and information will remain fully private and will never be used for promotional or educational purposes.

LIABILITY, INDEMNITY & DISCLAIMER​
​
By participating in services offered through Stargate Healing & Hypnosis, you acknowledge that all content, guidance, and techniques shared during sessions, communications, or supplemental materials are intended for informational and self-development purposes only. No guarantees are made regarding the effectiveness or outcome of services, as results may vary based on the individual. Under no circumstances shall the practitioners under Stargate Healing & Hypnosis, or any affiliates of, be held liable for any loss, injury, or damage of any kind, including but not limited to direct, indirect, incidental, consequential, special, or punitive damages, or any economic loss, arising out of or in connection with your participation in these services—whether based in contract, tort, negligence, or otherwise.

By booking and participating in services, you agree to fully indemnify, defend, and hold harmless any practitioner with Stargate Healing & Hypnosis from any claims, liabilities, costs, or legal expenses resulting from your use of the services provided, or from your failure to comply with this agreement or any applicable law or regulation.

GOVERNING LAW
​
This agreement shall be governed by and interpreted in accordance with the laws of the State of Texas, United States. By engaging in services, you consent to the exclusive jurisdiction of the courts in Texas for the resolution of any disputes arising out of or relating to these terms and conditions.

REVISIONS TO TERMS
​
Stargate Healing & Hypnosis reserves the right to update or amend these terms and conditions at any time, without prior notice. It is your responsibility to review any updates, particularly if returning for additional sessions after a period of absence. Continued use of services signifies your acceptance of any modified terms.
​
DATA PROTECTION
​
Your personal information is collected, stored, and used in compliance with Stargate Healing & Hypnosis’ Privacy Policy. By booking a session, you acknowledge and accept the terms of this Privacy Policy. If you do not agree to the policy, please do not proceed with booking services. Please note: Privacy terms and practices are subject to change without prior notice, so you are encouraged to review the policy regularly to stay informed of any updates
CONCERNS & COMPLAINS

​If at any point you have a concern or complaint about your experience, please bring it to our attention directly and promptly via email at stargatehealinghypnosis@gmail.com. Every effort will be made to address and resolve the issue in a professional and respectful manner.

STATEMENTS OF UNDERSTANDING
​
By booking a session, you acknowledge that you have read, understood, and agree to these Terms and Conditions.

I understand the scope of the services offered by the practitioners at Stargate Healing & Hypnosis and give full consent to receive therapy.

I acknowledge that results may vary, and no specific outcomes are guaranteed.

I understand that the therapy provided is not a substitute for medical, psychological, or psychiatric treatment.

I will consult a qualified healthcare provider if needed. If advised to seek medical approval before continuing therapy, I agree to obtain written consent from my doctor or healthcare provider.

I am free to end sessions at any time.

I understand my active participation and motivation are essential to the process, including following through on tasks or audio recordings suggested.

I confirm that all information I have provided is accurate and will continue to be truthful during future sessions.

I have read, understand, and agree to the cancellation, rescheduling, and no-show policies outlined in these Terms and Conditions, including any applicable fees or forfeiture of payments.
